Here's how to think about it.

On-demand tadalafil

Taken 30 minutes to a few hours before sexual activity, on-demand tadalafil typically uses a higher dose, often 10mg or 20mg. Its effects last up to 36 hours, giving a wide window without needing to time things precisely.

Tadalafil's long half-life of approximately 17.5 hours is what makes this extended window possible. A single 10mg or 20mg dose provides activity for roughly two days, which is part of why it earned the nickname "the weekend pill."1

This option works well for men who have sex infrequently or unpredictably and don't want to take a daily pill for something that doesn't happen daily.

Daily tadalafil

Daily tadalafil uses a much lower dose, typically 2.5mg or 5mg, taken every day regardless of planned activity. The goal is to maintain a steady level of the medication in your system so you're always ready.

Because of tadalafil's long half-life, steady-state plasma concentrations are reached after about 5 days of daily dosing. At that point, drug exposure is approximately 1.6 times higher than after a single dose. Researchers have found that five days of daily 10mg or 5mg dosing produces cumulative plasma exposure roughly equivalent to on-demand doses of 16mg or 8mg.1

This removes the planning element entirely. No timing, no waiting, no thinking about it before sex.

A systematic review and meta-analysis of randomized controlled trials comparing the two approaches found that daily tadalafil produced modestly better erectile function scores than on-demand dosing after at least 24 weeks of consistent use.2 Long-term safety data from a multicenter trial following patients for 12 months found once-daily dosing to be both safe and effective, with most men reporting meaningful improvement in erectile function over the treatment period.3

Which one fits your life

If you have sex a few times a month, on-demand makes sense. If you have sex multiple times a week, or you want spontaneity without planning, daily dosing often makes more sense.

Pharmacokinetic modeling suggests daily low-dose tadalafil is particularly well suited for men who anticipate at least twice-weekly sexual activity, since the steady drug levels match that frequency well.4

Cost can also factor in. Daily dosing means filling a prescription every month regardless of how often you use it. On-demand means you only take what you need, but some men find the unpredictability stressful.

There's no objectively correct answer. It depends on you and what you're looking for. And switching isn't a one-way decision; once you try one approach, you're not locked into it.
