Please read these Terms of Service ("Terms") carefully before using the services offered by Chris Meyer FNP PLLC, operating as ChrisFNP ("Practice," "we," "our," or "us"). By accessing our website or using our telehealth services you agree to be bound by these Terms. If you do not agree to these Terms do not use our services.
ChrisFNP is a telehealth practice offering asynchronous men's health services in the state of Arizona. Services are provided by Chris Meyer, FNP-BC (Arizona License No. 338352, NPI: 1417633314), a licensed Family Nurse Practitioner with full practice authority under Arizona law.
Current services include:
Finasteride is FDA-approved for male pattern hair loss. Dutasteride and oral minoxidil are prescribed off-label for hair loss; paroxetine is prescribed off-label for premature ejaculation. Off-label prescribing is a common and legal practice in medicine when supported by clinical evidence. The appropriateness of any medication is determined by Chris Meyer, FNP-BC, based on individual clinical assessment.
All care is delivered asynchronously via secure messaging through our clinical platform. No real-time video or in-person visits are offered or required.
Provide accurate and complete health information on your intake questionnaire
Not have a medical condition that contraindicates the requested treatment
By using our services you represent and warrant that you meet all eligibility requirements listed above.
ChrisFNP is a specialty telehealth service focused exclusively on men's hair loss, erectile dysfunction, and premature ejaculation. We are NOT your primary care provider and our services do NOT replace primary care.
You are responsible for maintaining a relationship with a primary care provider for your overall health needs. If you are experiencing a medical emergency call 911 or go to your nearest emergency room immediately.
Submission of a completed intake questionnaire does not guarantee that a prescription will be issued. Chris Meyer, FNP-BC, retains full clinical discretion to:
Decline to prescribe if the requested medication is not clinically appropriate
Clinical decisions are made based on the information you provide. You are responsible for providing accurate and complete health information. Providing false or incomplete information may result in an unsafe prescription and may constitute fraud.
Access to ChrisFNP clinical services requires a monthly subscription of $50.00 USD. This fee covers:
Prescription management and monthly refill processing
The $50.00 monthly subscription fee does NOT include the cost of medications. Medications are prescribed to your designated pharmacy and paid by you directly. We recommend Cost Plus Drugs (costplusdrugs.com) for the most affordable pricing.
Your subscription will be billed monthly on the date you enrolled. Subscriptions automatically renew each month until cancelled. Payment is processed through our HIPAA-compliant payment platform.
You may cancel your subscription at any time by contacting us through our secure messaging platform. Cancellation takes effect at the end of your current billing period. No refunds are issued for partial months. Upon cancellation your prescription will not be renewed the following month.
Active patients may participate in our referral program. Each patient receives a unique referral code after enrollment. New patients who use a referral code receive $25.00 off their first month. Referring patients receive one month free ($50.00 credit) for each successfully enrolled referral. Credits are applied to the following month's invoice. Referral codes are for new patients only and may not be used by existing patients to reduce renewal fees.
Prescriptions issued through ChrisFNP are valid for use at licensed pharmacies in the United States. Prescriptions are issued for a 30-day supply with no refills. Monthly refills require your active subscription and are processed automatically each billing cycle.
We prescribe non-controlled substances only. We do not prescribe controlled substances under any circumstances through this platform.
Prescriptions are transmitted directly to your designated pharmacy via secure electronic fax. You are responsible for providing accurate pharmacy information. We are not responsible for delays or errors caused by incorrect pharmacy information.
ChrisFNP is licensed to practice in the state of Arizona only. Services may only be rendered to patients who are physically located in Arizona at the time of the clinical encounter. If you relocate outside of Arizona you must notify us immediately and we will be unable to continue providing services until appropriate licensure is obtained in your state.
To the fullest extent permitted by applicable law, Chris Meyer FNP PLLC, its owner, agents, and representatives shall not be liable for:
Any indirect, incidental, consequential, or punitive damages arising from your use of our services
Any adverse medication effects that result from inaccurate or incomplete health information provided by the patient
Any delay in prescription processing caused by pharmacy error or incorrect pharmacy information
Any harm arising from your failure to disclose relevant medical history or current medications
Our total liability to you for any claim arising from your use of our services shall not exceed the total amount paid by you in the three months preceding the claim.
Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded under applicable law.
You understand that ChrisFNP delivers care asynchronously via secure messaging without a real-time video or in-person visit
You understand that telehealth has inherent limitations compared to in-person care including the inability to perform a physical examination
You understand that your care provider may determine that an in-person evaluation is necessary and may decline to prescribe without one
You consent to the transmission of your health information through HIPAA-compliant electronic platforms for the purpose of receiving care
You understand that technology failures may occasionally disrupt communication and that you should seek in-person care for urgent needs
Provide accurate, complete, and current health information at intake and upon any material change
Notify us promptly of any new medications, medical conditions, or adverse reactions
Seek immediate emergency care if you experience a medical emergency or serious adverse reaction
All content on www.chrisfnp.com including text, design, logos, and clinical materials is the property of Chris Meyer FNP PLLC and is protected by applicable copyright and intellectual property laws. You may not reproduce, distribute, or create derivative works from our content without written permission.
These Terms are governed by the laws of the state of Arizona without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of our services shall be resolved through binding arbitration in Maricopa County, Arizona, under the rules of the American Arbitration Association. You waive any right to participate in a class action lawsuit or class-wide arbitration.
We reserve the right to modify these Terms at any time. When we make material changes we will update the effective date and notify active patients via secure message. Continued use of our services after notification constitutes acceptance of the revised Terms.
We reserve the right to terminate your access to our services at any time for any of the following reasons:
Conduct that is abusive, threatening, or disrespectful toward the provider
Upon termination your prescription will not be renewed and you will be responsible for seeking care elsewhere. You will receive a copy of your records upon request.
