If you've been looking into ED treatment, you've come across two names more than any others: sildenafil and tadalafil. They're the generics behind Viagra and Cialis. Both work. Both are safe. The question is which one fits your life.

Here's how I think about it.

How they work

Both sildenafil and tadalafil are PDE5 inhibitors. They work by relaxing smooth muscle and increasing blood flow to the penis in response to sexual stimulation. Neither medication causes an erection on its own; arousal is still required.

The medications themselves are similar. The main difference is how you take them. Sildenafil is taken as needed before sexual activity, while tadalafil can be taken the same way or as a once daily low dose regardless of planned activity.

Both are well established as first-line treatments for ED, with strong evidence across large clinical trials and meta-analyses.1

Sildenafil

Sildenafil is taken on demand, roughly 30 to 60 minutes before sexual activity. Its effects typically last 4 to 6 hours.

Food affects sildenafil. A high-fat meal can delay absorption and reduce peak concentration, meaning it may take longer to work and be less effective if taken after a heavy meal. Timing around food is something men need to factor in.2

Tadalafil

Tadalafil has two distinct use cases.

On-demand tadalafil is taken 30 minutes to a few hours before sexual activity and lasts up to 36 hours. The long duration means less pressure around timing.

Daily low-dose tadalafil, typically 2.5mg or 5mg taken every day regardless of planned activity, maintains a steady level of the medication in the system. Men who choose this option don't have to plan around a pill at all. A meta-analysis of randomized controlled trials found that daily tadalafil produced greater improvement in erectile function scores compared to on-demand tadalafil after at least 24 weeks of treatment.3

Only the low dose is appropriate for daily use. Higher on-demand doses of tadalafil should never be taken daily; doing so increases the risk of more severe side effects without additional benefit.2

Food does not significantly affect tadalafil absorption, making it more flexible around meals.2

Side effects

Both medications share a similar overall adverse event rate. A large real-world study of over 130,000 patients found sildenafil and tadalafil had comparable side effect rates of approximately 35% and 34% respectively.4 Headache and flushing are more commonly associated with sildenafil. Reflux and muscle aches are more commonly reported with tadalafil, the latter likely related to its additional action on PDE11. These are generally mild and short-lived.4

Neither medication is safe to combine with nitrate medications. That combination can cause a dangerous and potentially life-threatening drop in blood pressure. This is screened for during your intake.2

So which one is right for you?

That depends on how often you anticipate needing the medication and what fits your lifestyle.

Sildenafil tends to suit men who want something taken as needed and prefer a shorter window of effect. Tadalafil suits men who want more flexibility around timing, or who prefer not to think about it at all with the daily option.
